Scope: every file under `docs/` + `README.md`.

## 1. Summary
Before Phase 0, **9 of 19** doc files were empty (0 bytes), the CHANGELOG was inaccurate, and several docs referenced files/folders that don't exist. Phase 0 populated all empty docs, corrected inaccurate ones, and synchronized structure references.

Documentation score: **4/10 → 7/10**.

## 3. Broken links / references (found & handled)
- `README.md` referenced `.github/` and `scripts/` (don't exist) → replaced with accurate structure + "planned" note. **RESOLVED**
- `AI_COLLABORATION.md` referenced `ARCHITECTURE.md`/`CODING_STANDARDS.md` (wrong names) → fixed to real paths. **RESOLVED**
- `ENGINEERING.md` folder list included `functions/`, `seed.sql` (absent) → annotated as planned. **RESOLVED**
- `DECISIONS.md` ADR-014 shows a folder tree with `scripts/`/`.github/` → left as historical ADR but flagged. **DOCUMENTED**

## 4. Remaining documentation gaps (OPEN — Phase 1)
- **Diagrams:** no C4/architecture diagram, no sequence diagrams, no ERD.
- **Onboarding:** no dedicated "getting started / local setup" runbook beyond README basics.
- **Deployment runbook:** `08_Deployment.md` describes target process but there is no executable pipeline.
- **ADRs for data model:** no ADR explains the actual tables / RLS choices; DECISIONS.md ADR-014 folder list is stale.
- **API contract:** service return shapes are inconsistent; `05_API.md` documents this but a normalized contract is not yet defined.

## 5. Duplication / consolidation
- Security/RLS/DoD prose is repeated across `ENGINEERING.md`, `AI_COLLABORATION.md`, `13_CODING_STANDARDS.md`, `AUTOMATIONS.md`, `DECISIONS.md`. Recommend a single canonical source with links (Phase 1). **DOCUMENTED**

## 6. Verdict
All intentionally-empty engineering documents are now populated and no document is empty. Documentation now reflects the current implementation, with remaining gaps (diagrams, onboarding, deployment pipeline, data-model ADRs) explicitly tracked for Phase 1.
